How do I obtain a
copy of my medical records?
You must fill out a Release of Information form and return it to us.
You may receive a form the following ways:
- Contact the Medical Records department at ext.253 and request that
a form be mailed to you;
- Download the form from the Forms
section of this site; or
- Come in to the Center and fill out the form in person.
Once the request is received, it generally takes 48-72
business hours to process your request.
How much will it cost to obtain a copy
of my medical records?
Your first copy is free. Each additional copy is $1.00 per page. It
is recommended that once you receive your initial (free) copy, you make
a copy for yourself in the event that you need a second copy.

What should I expect at
my first visit?
Your first visit is called a Pre-evaluation. This visit helps us to
obtain all necessary paperwork, medical records and information needed
prior to your first appointment with the physician. At this visit you
will be in a large group setting where you will fill out paperwork and
learn the policies of the Center. You will then be interviewed alone
with the Patient Coordinator. All personal information is discussed
one-on-one with a Patient Coordinator. After gathering all of your information,
the Patient Coordinator will have you scheduled to see the physician
that can best assist you with your pain problem. Because you will be
filling out paperwork, if you require assistance, please bring someone
to help you. Also, if you require glasses for reading and writing, please
bring them. Plan on this visit lasting a few hours.
Why can't I see a doctor at the first visit?
To have the most beneficial New Patient Evaluation with your doctor,
it is necessary to have all paperwork completed and all necessary medical
records available. The pre-evaluation also lets us make you aware of
our Center's policies and allows us to triage you to the most appropriate
physician for your specific pain problem. Because you are not seeing
a doctor at your first visit, you are not considered a patient at our
Center until this is established by the physician at the appointment
with him/her. We let your referring physician know that your first visit
is a pre-evaluation and, therefore, no treatment was initiated (i.e.-
no medications were prescribed, no tests were ordered, etc.).

What should I do if
I need assistance with my implanted drug delivery system or spinal cord
stimulator after business hours?
There is always a physician on-call, just call 859-278-1316 which will connect you to our
after-hours emergency line. This will allow you to speak with our on-call physician.
How often does an implanted drug delivery
system need to be refilled with medication?
Each patient will have a different refill interval depending on the
amount of medication your system holds, dose per day and concentration
of medication.
Would I be eligible for an implanted
drug delivery system if I am allergic to Morphine?
Yes, there are several other medications that may be used in the system.
How long does a refill procedure take?
The refill procedure takes approximately 30-40 minutes.
